AN OPEN INVITATION TO ANY DRIVER MANAGER PAST OR PRESENT
I WOULD LIKE YOU TO MAKE AN OPINION ON THIS SCENARIO USING ALL YOUR RAILWAY EXPERIENCE AS A DRIVER MANAGER AND EXPLAIN WHAT TYPE OF DISCIPLINARY ACTION (IF ANY) YOU WOULD TAKE AGAINST A DRIVER.
A driver sets off from point A, travelling to point B, which is about 95 miles away, the weather conditions are minus 2. Approximately 10 miles into this journey the driver encounters a partial failure of the warning horn. This is a common fault on the traction unit he is driving.
He arrives at the first of two station stops, he is running late and on green signals, he attempts to use the NRN radio to contact his control (and starts to fill out the repair slip) but the call fails and he receives the ready to start from the conductor.
At the second station stop he is again running late and on green signals, but also stopped across a busy junction, he tries to contact his control (filling out the repair slip at the same time) using the NRN radio and once again the call fails, he then gets the ready to start from the conductor.
During the remainder of this journey due to a rise in temperature his warning horn becomes fully operational, he receives confirmation from his conductor that the train is terminating at his destination and being used to form a different service which will be driven from the other cab.
On his late arrival at his destination he has approximately 17 minutes before his next service departs, he rips the partly filled in repair slip out of the book not realising he hadn`t filled in his details such as name, grade, time etc, and puts it into his drivers bag, he needs the toilet and a drink, however he is further delayed because he helps passengers who have just started boarding this train remove their baggage because they have not been informed about the set swap.
REGARDING THE RULE BOOK AND A COMPANY'S CONTINGENCY PLANS THAT MIGHT APPLY IN THE REPORTING OF A PARTIALLY DEFECTIVE WARNING HORN, WHAT (IF ANY) DISCIPLINARY ACTION WOULD YOU TAKE AGAINST THIS DRIVER AND WHY?

I agree totally, I could very well have wrote this, but unlike some, I don`t have to lie, the numerous people I have spoken with on this subject all say the same.
