"A message for our readers", interesting incident, I wonder if this train travelled at caution on its way back to York? Logic and common sense would suggest that this would have happened for a variety of reasons?

It shows the concerns of train crew regarding the warning horn problems, it also shows an admittance that warning horn failures occurred during the 2009/2010 winter and says the department took steps to investigate the reasons behind these failure modes. What happened from 2006 onwards? Is it just the case that every year we can say, we are investigating the failures?

So testing was done which took into account, wind speed, temperatures up to -10, conditions such as fog, snow and ice.
Great, but surely the question has to be, why did these units come into service without being thoroughly tested in the first place, after all, a delegation from the union ASLEF and Driver Managers from TPE went to Germany and saw these units on a test track. Questions should have been prepared and asked, yet not one health and safety representative was invited.
The conclusions. recommendations which all go towards the performance should have been sorted before leaving Germany.

The small piece at the bottom of page 10 and top of page 11, "Incident Reporting". "Your assistance in this matter is of high value and extremely important", obviously between 2006 and 2009 this was of no importance.
What all of this shows is that up until December 2011 there are still problems with these warning horns, the game of Russian Roulette is still being played out!
